This includes the start of a regional community health assessment centered on the Vital Conditions\, a community-led grants process\, the launch of a prenatal-to-early childhood initiative\, and the relational work needed to keep leaning in together. \nThe Contra Costa Together 2026 Summit will bring community-based organizations\, residents\, and systems together to strengthen relationships and our collective ability to engage\, collaborate\, and take action to advance the Vital Conditions that allow all people and communities to flourish. \nThis is not a traditional conference.
